Summary:IntroductionVentricular tachycardia (VT) and ventricular fibrillation (VF) leading to sudden cardiac death remains responsible for significant morbidity and mortality in patients with cardiac sarcoidosis. Although 18F-fluorodeoxy-glucose (FDG) uptake in left ventricle on positron emission tomography/computed tomography (PET/CT) is well studied, FDG uptake in right ventricle has not been elucidated in detail. The aim of this study was to evaluate the FDG uptake in right ventricle and its relationship to ventricular arrhythmias (VAs) in patients with cardiac sarcoidosis.MethodsWe retrospectively investigated 80 cardiac sarcoidosis patients. All patients underwent echocardiography and PET/CT. Serum angiotensin converting enzyme (ACE) and B-type natriuretic peptide (BNP) levels were also evaluated.ResultsOf the studied population, 19 patients (23.8%) had VAs, including 17 VTs and 2 VFs. PET/CT demonstrated positive left ventricular uptake in 37 patients (46.3%), right ventricular uptake in 19 patients (23.8%), and 15 patients (18.8%) had bilateral ventricular uptake. Nine patients with VAs had right ventricular FDG uptake, whereas 10 patients without VAs had right ventricular FDG uptake (47.3% vs 16.4%, P = 0.011). Multivariate analysis revealed a significant association between VAs and left ventricular ejection fraction (odds ratio [OR]0.96; 95% confidence interval [CI]0.93-0.99, P = 0.015), right ventricular FDG uptake (OR3.46; 95% CI1.06-11.3, P = 0.040). Meanwhile, age, gender, serum ACE and BNP levels, and left ventricular FDG uptake were not associated with VAs.ConclusionsRight ventricular FDG uptake was relatively common in patients with cardiac sarcoidosis, and independently associated with VAs.
