Summary:The STBM National Strategy has an outcome indicator, namely a decline in the incidence of diarrheal diseases and other environmental-based diseases related to sanitation and behavior. The purpose of this study was to determine the 5 Pillar Community implementation Based on Total Sanitation Program (STBM) in the South Nias District. The study was a quantitative by a cross-sectional approach. The population in the study was people in the South Nias Regency area of 290 families using total sampling. The results showed that the majority were not good at open defecation as many as 123 families (42.2%) with a significance value of 0.001, the majority were not good at washing hands with soap as many as 143 families (49.3%) with a significance value of 0.003, The majority are not good at managing household drinking water and food as manyas 125 families (43.2%) with a significance value of 0.000, the majority are quite good at securing household waste as many as 136 families (46.9%) with a significance value of 0.002, the majority are quite good at securing household liquid waste as many as 121 families (41.7%) with a significance value of 0.001, Community-Based Total Sanitation Factor (STBM) has a significance value of 0.002
