Summary: | In this introduction to the special issue on The Persistence of the Health Insurance Dilemma, the guest editor establishes the thematic questions for the journal's case studies. A brief history of the development of systems of worker's medical insurance & provision of health services in Canada & the US addresses the political of European systems & their influence on American reformers. The editor presents the questions of why increased growth in government responsibility did not occur, & the correlation of reduced income due to injury to the sharp expenditure increase on medical goods & services in the case studies of the US, England, & South Africa. The restriction of the journal perspective to the past is asserted to offer clearer distinctions regarding the importance of culture, economics, & race in the cases of early twentieth century American is concluded to access the rich historical literature that studies of the politics behind the establishment, co-option, or rejection of universalist government health insure or health services provision while emphasizing the on-the-ground effects of health insurance.
