Summary:A 58-year-old woman with a history of an ovarian fibroma, presented 18 months after surgery with recurrent ascites and hepatic encephalopathy. Ascitic fluid protein was 52 g/L and the serum to ascites albumin gradient (SAAG) was 15 g/L. Liver biopsy showed centrilobular sinusoidal dilation and hemorrhage consistent with passive congestion and absence of cirrhosis. Despite a normal echocardiogram, cardiac catheterisation revealed findings consistent with constrictive pericarditis.
