Summary: | • Necrotizing fasciitis in the early stage can look like cellulitis. Pain is out of proportion to the exam, and tachycardia is usually present. Characteristic blisters and subcutaneous air are late findings. • Retropharyngeal abscess is a disease of young children, as the retropharyngeal lymph nodes involute around age 5 years. It presents with fever and sore throat. Trismus and reluctance to flex the neck are seen. X-rays of the cervical spine reveal increased paravertebral space at the upper cervical vertebrae. • Intussusception is the most common cause of bowel obstruction in children. Most often, it presents with abdominal pain and vomiting. However, in small children, it may present with just lethargy and decreased feeding. Ultrasound is more sensitive than radiographs. • Hair tourniquets are a can’t-miss diagnosis. These usually occur in infants, and the presentation often is just persistent crying. Magnification may be needed. |
